As a courtesy to you, our staff will complete the dental portion of the claim form and submit it to your carrier directly. To expedite processing, make sure that your part of the form is filled out.
When you assign benefits to our office, please ask our staff to estimate your coverage, you will then be asked to pay only a portion of the charges. We will then submit and wait for payment from your insurance carrier. Once we receive all insurance payments, we will then bill you for the remaining balance.
